Instructions for buying health insurance cards
1/ How to participate in household health insurance:
- REGISTER TO JOIN THE CLINIC:
NEW BUY (Customer has never participated in health insurance or converted from other card subjects to Household)
– Customers need to provide documents: Citizen identification card with chip, household registration book (if any), temporary residence registration for first-time participating customers with permanent residence outside Dong Nai province, In case the new customer has a household registration outside the province, a temporary residence registration certificate will be provided.
– Card validity: 30 days after purchase
– Staff will contact to return the card to the customer when the card is available.
RENEWAL (Customer is using Household health insurance card)
– Customers need to provide documents: ID card, ID card (if any) and bring household registration book (if any) for household purchases
– Card validity:
+ Continue the expiration date for unexpired cards
+ 14 days after payment for cards expiring within 90 days
+ 30 days after payment for cards expired more than 90 days
– Staff will contact to return the card to the customer (if any)

2/ Voluntary health insurance
- OBJECTS PARTICIPATING IN VOLUNTARY HEALTH INSURANCE
Health insurance participants include:
(1) Groups are made up of workers and units.
(2) Group paid by social insurance organization.
(3) Group paid for by the state budget.
(4) Group whose contributions are supported by the state budget.
(5) Employer-owned group.
(6) Group of health insurance participants by household.
Groups participating in health insurance by household include:
– All people named in the same household register for permanent residence, except for people in categories 1, 2, 3, 4, 5 and people who have declared temporary absence.
– All people named in the same household registering for temporary residence, except for people belonging to subjects 1, 2, 3, 4, 5 and subjects participating in health insurance under regular registered households residence.
– The following subjects are eligible to participate in household health insurance:
+ Dignitaries, officials, monks;
+ People living in social protection facilities without support from the state budget to pay health insurance.

3/ Amount of household health insurance payment
– The monthly payment amount of a household health insurance participant is determined by the health insurance premium multiplied (x) by the base salary at the time of health insurance payment;
– When the State adjusts the health insurance premium or adjusts the base salary, participants do not have to make additional payments or receive a refund of the difference due to the adjustment of the health insurance premium or salary. base salary for the remaining time the participant has paid health insurance.
